By completing this Letter of Appointment, you authorise Elliott Australia Group Pty Ltd (AFSL 418512) trading as All Trades Cover to act on your behalf as your general insurance broker. This allows All Trades Cover to liaise with insurers, review your current insurance position, request relevant policy and claims information, and assist with the ongoing management of your general insurance needs.

Where you already have a broker in place, this appointment will allow All Trades Cover to take over servicing of your existing general insurance arrangements.

4.0 Important Documents & Acknowledgement

Please read each of the following documents before signing below.

Whilst this Letter of Appointment is not on any letterhead, I attest to its authenticity and intent in my capacity as authorised representative of the named insured.

By signing this Letter of Appointment, I confirm that I am duly empowered and authorised to sign on behalf of the named insured listed above.

Effective from the signed date, I appoint Elliott Australia Group Pty Ltd (AFSL 418512) trading as All Trades Cover, as our insurance broker and authorise All Trades Cover to act on our behalf in relation to the insurance policies listed in this form and any related insurance arrangements.

I authorise our current and former insurers, underwriting agencies, premium funders and insurance intermediaries to provide All Trades Cover with all information requested regarding our insurance policies, claims history, premium history, renewal terms, underwriting information and related documentation for the past five (5) years.

In the event that All Trades Cover receives any personal information from an insurer or other party, All Trades Cover is required to handle that information in accordance with its Privacy Policy.

This appointment replaces any existing arrangement in place with any other insurance intermediary in relation to the policies listed in this Letter of Appointment.
